Apply for Benefits

The applications below will allow you to submit your documents electronically. If you have a smart phone there is an upload feature built into the application that allows you to take a picture image of your documents.  The application process is easy and will take approximately 10 minutes to complete and upload your documents.

 The following documents should be submitted with your application:

  1. Notice of Investigation
  2. Notice of Discipline/Waiver
  3. If applicable, FRA Certificate of Suspension and Certificate of Revocation (Revocation must have a start date and end date)
  4. Transcript of Investigation, including Exhibits, if discipline exceeds thirty (30) days
